The short answer
Take the removable mats out and wash them separately. On the fitted carpet, scrape the solids off, blot hard with kitchen roll, then use equal parts white vinegar and warm water applied on a cloth. Use as little liquid as possible, because underneath the pile is thick moulded foam that takes days to dry, and leave the doors open until the carpet is dry to the touch.
If it has just happened
Get the removable mat out of the car with the mess on it, then blot the fitted carpet underneath before anything soaks through. In a footwell everything runs downhill and collects at the lowest point, which is where the underlay is thickest.
Heat can set this stain permanently
Do not use hot water, a tumble dryer, an iron or a steam cleaner until the mark has gone. Heat fixes this type of stain into the fibres and makes it far harder to shift.
Test on a hidden area first
Try any cleaner on a hidden part of the material first, such as a seam, an offcut, inside a hem or under a piece of furniture. Wait for it to dry and check for colour change before treating the visible mark.
Cleaning the mark is not the same as disinfecting
If the sickness came with a stomach bug rather than an ordinary posset, the surface may carry norovirus, which spreads easily and survives on hard surfaces. UK public health guidance is to wear gloves, avoid shaking contaminated laundry, wash affected laundry separately with normal powder at 60 C where the care label allows it, and disinfect compatible hard surfaces with a bleach based product afterwards. Do that after the stain work, not instead of it, and only where the material tolerates it.
Keep every kind of drying heat away until the mark has gone
Baby sick is mostly milk protein. Heat coagulates protein and locks it into the material, which is why a mark that would have washed out can become permanent after one hot wash, one pass with an iron, a hairdryer or a spell on a radiator. Cool or warm treatment, air drying, and a proper look in daylight before anything warm goes near it.
A car is a small sealed box and the smell is the real problem
A house dilutes an odour. A car concentrates it, then bakes it every time the sun comes out, and warm damp upholstery in a closed vehicle grows mould. Whatever else you do, get the material out, get the moisture out and leave the doors open until the trim is dry to the touch. A car that still smells after a week almost always has liquid somewhere you have not looked, usually under the seat rails or in the foam beneath a seat cover.

- A vacuum, ideally with a crevice tool Cars are all seams and gaps, and that is where it goes.

Lift the mats out
Take the removable mats out of the footwell with the mess still on them and put them on the drive. They can be scrubbed and hosed later without any of the constraints that apply inside the car.

Use a spoon working from the edge of the mark inwards, then press kitchen roll down hard. Firm pressure lifts liquid out of the pile before it reaches the foam, and that is the whole game here.

Press dry kitchen roll in hard, then leave the doors or windows open and the blower running on cold until the carpet is dry to the touch.

Break it up and vacuum it out
Work the crust loose with a blunt scraper and vacuum with the crevice tool. Get right into the edges where the carpet meets the sill and the transmission tunnel.

Rehydrate on the cloth, not in the carpet
Vinegar and warm water on a white cloth, pressed on for a minute to soften what is left. Open the doors first, because rehydrating dried sick brings the smell straight back.

Work the damp area with a soft brush in one direction to lift the residue out of the base of the pile, then blot immediately.

Leave the car open with the fans running until the carpet is dry, then look at it again. A second pass on damp carpet only adds water.

Vacuum everything, including the seat rails
Old sick works into the runners, under the seat and along the sill. Use the crevice tool everywhere you can reach before deciding the carpet is the problem.

Bicarbonate of soda overnight, repeatedly
Cover the dry carpet, leave it overnight with the car shut, and vacuum it out in the morning. Repeat for several nights. This reaches the odour without adding any moisture.

Air it properly whenever you can
Windows open when the car is parked somewhere safe and dry, and the blower on cold with recirculation off while driving. Sealed and warm is the condition that makes it worse.

Then get it extracted professionally
A valeter with a wet extraction machine and good recovery can pull liquid out of the underlay. Ask them to lift the carpet where they can, because cleaning the top of it does not reach the layer holding the smell.
What not to use on car carpet

Soaking the footwell or using a spray bottle freely
Whatever soaks through the pile lands in moulded foam glued to the floor pan, where it cannot dry. That is how a stain becomes a smell that lasts months.

Bleach and oxygen bleach powders
Car carpet is dyed to match the trim and cannot be patched. A pale patch in a footwell is permanent and impossible to match.

A steam cleaner or the car's own heater aimed at the wet patch
Heat sets milk protein, and steam puts more water into the layer that cannot dry. Both make the outcome worse in different ways.

Strong solvent cleaners
Carpet manufacturers exclude aggressive solvents, and in a car they can also attack the adhesive holding the underlay to the floor pan.
White compared with coloured car carpet
Car carpet is dyed to match the rest of the interior and cannot be patched, so anything that lightens it is a permanent mismatch in a small enclosed space where you will see it every day. That rules out bleach and oxygen powders entirely, and it is why the method here stops at mild acid and mechanical removal.

If the mark is still there
Smell is what survives in a car, and it comes from the underlay rather than the pile. If the carpet looks clean and the car still smells after a few days, liquid is in the foam beneath it.
The things that actually help are airflow and time. Leave the windows open when the car is somewhere safe and dry, run the blower on cold with the recirculation off, and put a container of bicarbonate of soda in the footwell overnight, several nights running.
If it still smells after a week, or if you can press the carpet and feel dampness under it, the carpet needs lifting. On most cars that means removing the seats and the sill trims, which is a job for a valeter with a wet vacuum. Ask for a hot water extraction with strong recovery rather than a shampoo, and ask them to check under the carpet rather than only cleaning the top of it.
When to stop and call someone
Get a car valeter with a wet extraction machine in rather than continuing if the carpet is still damp underneath after a couple of days, if the smell persists after a week of airing, or if the sick went down the side of the transmission tunnel or under the seat rails where you cannot reach. Ask them to lift the carpet rather than only cleaning the surface.

A car footwell is the lowest point of a sealed metal box, lined with a sponge.
Two layers, two drying times
The pile you can see is thin and dries in an hour. Under it is moulded foam or felt several centimetres thick, glued to the floor pan, which dries in days if it dries at all. Liquid that gets past the pile is effectively out of reach.
Why so little water
Every extra millilitre you add has to come back out through the same thin pile. Carpet manufacturers warn that over wetting leaves detergent and fine soil behind, causing rapid resoiling, and in a car it also produces the damp smell that people notice long after the mark has gone.
Why vinegar and not something stronger
Diluted white vinegar sits inside the mildly acidic to mildly alkaline range that carpet manufacturers specify for spot cleaning, it handles the sour smell of milk residue, and it will not lighten the dye. Stronger cleaners buy very little here and risk the colour.
Why nothing warm
Baby sick is largely milk protein and heat sets protein. That excludes hot water, steam and the temptation to point a heat gun or the car's own heater at a wet patch.
